A fundamental aspect of our Foundation of Care is to continually strive to improve the working relationship between the long-term care provider and the acute care community.

We understand that assisting the long-term facility in caring for more clinically complex residents makes it more attractive to hospitals and health plans. That's why, in addition to assisting with direct patient care, PHS providers act as liaisons, vigilantly working with area hospitals to assure continuity of care for the patient.
